CHAPERONE POLICY

The Surgery is committed to providing a safe and comfortable environment where patients and staff can be confident that best practice is followed at all times and the safety of everyone is of paramount importance.

All patients are entitled to have a chaperone present during any consultation, examination or procedure where they feel one is required. The chaperone may be a family member or friend. On occasions you may feel that a formal chaperone is required – i.e. a trained member of staff.

Your GP or Nurse may also require a chaperone to be present for certain consultations in accordance with our Chaperone Policy.  Please download a copy of our Chaperone-Policy for further information.

Policies and procedures

If you have had confirmation that you can register with the surgery you may print your own forms to complete at home by using the following:-

If your records are with a GP surgery within the UK use an HS 200 Form HS200 to register when your records are held with a UK GP Surgery

If you do not currently have a GP in the UK then you must use an SCR1 form HSCR1 for those registering from outside of the UK
